not that i know of and i hope to never have it happen, although i lost weapon sparring to a lower belt once... (1 years later i got my revenge when he got into my belt division  )
have you ever tried to attack someone and stop an inch from them to scare them, but end up actually hitting them and hitting them quite hard...?

Not recently thankfully, it's so embarrassing when that happens. Once when i was working i walked into a lamp-post while staring at someone and he saw the whole thing lol.
