I had an instructor (back when I was younger and more naive) who years earlier had been kicked from his organization where he earned his rank, due to behavior. He had the certificate and the belt though, and I never thought to question his certification. I was only after I had a new instructor that I found out about this.

The organization officially removed him of his rank, but they did not take back his certificate or belt.
